Assessment, treatment, referral and care coordination for adolescent, adult and older homeless individuals in partnership with local services. Homeless Health Outreach offers a range of clinics at a variety of drop-in centres, services and churches. We also have a DENTAL CLINIC. Working in partnership with Way2Home Support Team (Neami), provides healthcare and support to young, adult & older rough sleepers with a long history of homelessness and multiple health needs. Service workers are sensitive to the cultural needs of people from culturally & linguistically diverse (CALD) backgrounds and those of Indigenous clients.
